Children and minors cannot directly use the website nor submit information. All child information and interaction is strictly under parental guidance.

  • Payment and billing information, such as billing address, payment status, insurance-related information if applicable, and records of services provided. We do not intend to store full payment-card numbers because payments are processed through a third-party payment processor.

If you choose to receive text messages from Eyes Up Reading, we may send appointment reminders, scheduling messages, service-related notices, responses to your questions, or other communications for which you have provided consent.
